Maintaining continuity of service includes:

• Providing appropriate home care services to patients/clients.
• Educating organizations and providers about home care services.
• Identifying major referral sources and establishing communication links.
• Developing an awareness of and integrating complementary services provided by other organizations and agencies.
• Working collaboratively with other organizations providing service.
• Referring appropriately to other organizations.
• Ensuring relevant information is communicated in a timely manner.
• Working with families/supporters to achieve continuity of service.
